This page has been developed to provide members a quick and easy way to give Mohave feedback about our vendors. Both positive and negative feedback is requested.

     


Types of Reports   There are three types of vendor reports which can be submitted, a description of each type is provided below:

Positive Performance: This type of report should be used to give any positive feedback you have regarding a Mohave vendor. If the vendor has done a great job, gone "above and beyond the call of duty," or provided any other positive service or support, please take a moment to tell us about your experiences.

Notice of Concern: This type of report should be used to voice any general concerns you may have about a Mohave vendor. This report lets us know that you encountered a problem or issue that Mohave should be aware of, even if no further action is required.

Request for Assistance: This type of report should be used to explain a specific problem with a vendor. Space is available to note the problem, any action that has been taken by the vendor, and what needs to be done to resolve the issue.

Please provide your contact information when filling out the report. Without contact information, we will be unable to respond.

When filing a "Notice of Concern" or "Request for Assistance," please provide as much information as possible regarding the vendor contact with whom you had discussions about the problem or concern. Also provide concise details on the issue(s).

Mohave will review and respond to all Vendor Performance Reports. If the report is a "Notice of Concern" or a "Request for Assistance," a Contracts Team member will contact you as quickly as possible for further information and/or details.

Mohave takes all vendor reports very seriously. We strive to have quality vendors providing quality products and services for our members. These reports will assist us by providing one measure of vendor performance when reviewing contract renewals and offers for new contracts.
