SAMPLE USAGE AND RECONCILIATION REPORT
Below is a sample of the reconciliation report. Contractor will be required to provide a Usage and Reconciliation Report detailing activity under an awarded contract and make payment for Mohave administration fees for invoices paid in the previous month. Payment and report will be due as per a schedule agreed upon by Mohave and Contractor. The initial due date shall be the 10th, 20th or 30th of the following month and will be specified in an award notification letter.

Proposals shall be delivered to Mohave Educational Services Cooperative, Inc., 625 E. Beale Street, Kingman, AZ 86401.
