Check the Member List to see if your organization is already a member. If your organization is not a member, download and print a copy of the Cooperative Purchase Agreement (Rev. 8/28/10).

SUBMIT AGREEMENT FOR ACCEPTANCE

Complete, sign, and mail the Cooperative Purchase Agreement to Mohave Educational Services Cooperative, Inc., 211 N 7th St, Kingman, AZ 86401. Agreement with original signatures is required for our files.

Once the agreement is signed by Mohave’s representative, you will receive a membership packet containing a letter of acceptance, a copy of the agreement for your records, and other informative literature.
